Note: Only one Program/AV Information Form is required for each workshop. Please be sure to note the primary contact on this form. All presenters for your workshop should have already submitted a Presenter Information Form. If not, please have them do so immediately. Deadline to submit is May 14, 2019

We will allocate other requested PTA equipment (overhead projector, chart pads/stand, etc.) to presenters if available and requested in advance. We will have internet access in all classrooms. Please indicate below any additional equipment you will need to do your presentation. Speakers and all sound equipment costs Florida PTA additional monies. Please be mindful when making your presentations. If you have a lot of sound in your presentation and need this additional equipment, please indicate in the "other equipment" section of this form.
