501(c)(3) Determination Letter

Please request a 501(c)(3) letter for any of the following:

1 – Legal Banking Letter (needed to open an account);
2 – Form DR5 (Florida Sales Tax Exemption Certificate);
3 – Standard 501(c)(3) letter for donation requests;
4 – Stripe Account (formerly WePay) through Givebacks (f/k/a MemberHub).
